Patient Centered Programs Working According to Horizon Blue Cross

Horizon Blue Cross completed a study in 2013 on the effectiveness of their patient center programs in lower cost of care and improving coordination of care. The results show that these programs are working!
Here are some of the results from the study, comparing members in a traditional primary care practice and those participating in the Patient-Centered Medical Home Program. Members in the Patient Centered Program had:
• 14% higher rate in improved diabetes control
• 12% higher rate in cholesterol management
• 4% lower rate in Emergency Room visits
• 2% lower rate in hospital admissions
• 4% lower total cost of care
Members in the patient centered practices were also able to avoid more than 1200 ER visits and 260 hospital admissions which represent a savings of about $4.5 million. For more information on this survey and the patient-centered programs, visit www.horizonblue.com.
