Independence Blue Cross (IBC) partners with ReAdmissions Solutions to offer Hospital Transition Program for Heart Failure Patients

In an effort to decrease hospital readmissions for patients with a primary diagnosis of congestive heart failure (CHF), IBC has partnered with ReAdmission Solutions to offer hospital to home transitional care assistance program.
With the program, eligible members who enroll will receive:
Home health visits by nurses, if medically necessary, in accordance with your health benefits plan
Follow-up support from ReAdmission Solutions clinical staff
Personalized education and the opportunity to ask questions
Eligible members will receive notification of the program in the mail and will be contacted by ReAdmission Solutions to sign up, members can also reach out to ReAdmissions directly at 1-800-533-3755.
The program is designed to work in collaboration with hospitals and providers, currently the following hospitals and/or health systems will be participating in the program: Jeanes Hospital, Temple University Hospital, Cooper Hospital, Einstein, Holy Redeemer Hospital, Jefferson Health System, Cardiology Consultants of Philadelphia, and Main Line Health System, which includes Paoli, Riddle Memorial, Lankenau, and Bryn Mawr hospitals.
