Each quarter Independence Blue Cross makes changes to its prescription drug formulary listing and procedures for safe prescribing. Effective October 1, 2015 several changes will go into effect, including those listed below. Compound drugs, regardless of their components will be moved to the non-preferred tier as of October 1, 2015. This may result in increased co-pays for those taking these medications. Also, any compound medication that costs $75 or more will now be subject to prior authorization requirements.
In addition to the change with compound medications, the following drugs will now require prior authorization:
Pennsaid 2% drops
In addition, the drugs below will now be subject to quantity limits:
Dilaudid 1mg/ml liquid
Tussionex ER susp
